Can I send people to a thank-you page after they submit a form?

I want to track form submissions as conversions with a separate page.

Yes. Open Contact Forms in the Settings sidebar, click the settings icon on the form and set a Redirect URL, for example a thank-you page you created under Pages. After a successful submit the person is sent there. If you leave it empty, they stay on the form and see your Success message instead.
